Here is the information you will need to complete the Capability Assessment

The Support Coordinator will schedule your appointment for the Capability Assessment once they have ensured that you meet the eligibility criteria to receive Assured Income and/or AccessAbility Supports. The Capability Assessment will be done at your home. Based on your functionality and your current unmet needs, the support you will require through AccessAbility Supports will be decided.
