Here is the information you will need to meet with a Support Coordinator to complete a Support Needs Assessment

During your appointment with a Support Coordinator, they will ask you questions about the supports you currently have and discuss your unmet needs. The Support Coordinator will explain details about the program funding. If you are approved by the Support Coordinator, a referral will be made to a program Assessor to complete a Functionality Assessment.

If you are not approved by the Support Coordinator, you have the option to appeal the decision. You can learn more about the appeals process in the section titled, After you apply - approved or denied, “What happens if your application is denied?”

If you are Indigenous there are many things that affect the benefits you can get and the dollar amounts you can get from them. These include the agreements your band or governing body has with provincial, territorial, and federal governments. Before applying to any benefits, you should speak with your governing body, if applicable.
