Did you send your completed CPP-D application form and completed medical report to Service Canada?

The application for disability benefits can be submitted separately from the medical report. Do not wait for your doctor or nurse practitioner to complete the medical report before sending your completed application. The date Service Canada receives your application could affect when your benefits start.
There are 2 ways to submit the application:
- You can apply for CPP-D online (You must also complete, sign, and mail a Service Canada Personal Information consent form. Mail it to your nearest Service Canada office.
- You can apply using a paper application.
The Medical Report may be sent to you from your doctor’s office. Or they may send it directly to Service Canada. If they send it to you, there are 3 ways to submit it:
- You can log into your My Service Canada Account (MSCA) and upload the medical report.
- You can send your CPP-D medical report by mail.
- You can drop it off at a Service Canada office.
If you have supporting documents for your medical conditions, include copies with the medical report. Some examples of supporting documents are a specialist’s report, a medical investigation report, a hospital discharge report, or longitudinal clinical notes.
Read the General Information Guide included with your application. It outlines the steps you need to take to ensure that Service Canada receives all the necessary information. To avoid delays, make sure that you do not have any missing information or sections. Sign and date all forms where you are asked to.
If you are mailing your application, write your Social Insurance Number at the top of each page.
Keep a copy of your completed application in a safe place. You may need to refer to it later.
You can send in your form at any time during the year.

If you are submitting your application online: To avoid delays, please ensure that you have completed the full application and that you do not have any missing information or sections.
If you are mailing your application:
- Indicate your Social Insurance Number on all the pages
- Sign and date all forms
- Enclose the Medical Report from your physician if the doctor has not submitted their report separately.
- If your children do not have a Social Insurance Number, enclose proof of birth (certified copies are acceptable).
Keep a photocopy for your records.
If you are submitting your application in person:
- Go to your local Service Canada office
- If you have children and they do not have a Social Insurance Number, bring proof of their birth to the appointment with you.
