Here are the steps you can follow to apply for Canada Pension Plan–Disability benefit. Click ‘Start step 1’ at the bottom of this page. This will take you to step one. Keep track of the steps you have completed by printing this page (select print from your browser) and checking off each step as you complete it. Go ahead to the step where you would like to continue.

You can go through the steps at your own pace. If you need help at any step, go to the Help page to find out where you can get support with your application.

  1. Do you have the CPP-Disability application form?
  2. Did you fill out the CPP-D application form?
  3. Did you fill out sections 1 and 2 of the medical report?
  4. Did you ask a doctor or nurse practitioner to fill out sections 3 to 9 and sign the medical report?
  5. Did you send your completed CPP-D application form and completed medical report to Service Canada?

If you are Indigenous there are many things that affect the benefits you can get and the dollar amounts you can get from them. These include the agreements your band or governing body has with provincial, territorial, and federal governments. Before applying to any benefits, you should speak with your governing body, if applicable.
