Here is how you can find out which sections you and the medical practitioner must complete.

The medical report is the most important part of your application.

Select the form that applies to you:

You must fill in sections 1 and 2 yourself. Write your Social Insurance Number at the top of each page. These pages ask for personal information about you. This is also where you give permission to share personal health information related to your application. This is because Service Canada will likely need to gather more information.

These include the agreements your band or governing body has with provincial, territorial, and federal governments. Before applying to any benefits, you should speak with your governing body, if applicable.
