Step 7 - Persons with Disabilities designation (BC)

Have you checked all the items on the Applicant Checklist in your application?

Here is what you need to do to check your application is completed.

When your application is completed, look at the Applicant Checklist at the beginning of the form. Make sure that you have included everything.

Be sure to make a photocopy of your completed application form. Keep this in a safe place with your other disability records.

Questions from the current Applicant Checklist

Have you completed Section 1 – Applicant Information?
Have you read and signed Section 1C – Declaration and Notification?
Has Section 2 – Medical Report been completed and signed?
Has Section 3 – Assessor Report been completed and signed?
Did you keep a photocopy for your records?
Did you remember to include any additional information you want considered?
Has proof of legal authority to act on behalf of the applicant been attached?

If you want the Health Assistance Branch to confirm it got your application, check the box, and put your name and address in the space for this under the checklist.
