Here are the steps you can follow to apply for Persons with Disabilities (PWD) status. Click ‘Start step 1’ at the bottom of this page. This will take you to step one. Keep track of the steps you have completed by printing this page (select print from your browser) and checking off each step as you complete it. Go ahead to the step where you would like to continue.

You can go through the steps at your own pace. If you need help at any step, go to the Help page to find out where you can get support with your application.

Read the application and think about how your functional abilities and daily living activities are restricted and how you might need help with these activities.

  1. Do you receive Income Assistance?
  2. Do you have a Persons with Disabilities (PWD) application form?
  3. Have you filled out the applicant’s section of the form?
  4. Have you read and signed Section C – Declaration and Notification?
  5. Have you given your application to your medical professional to complete the Medical Report?
  6. Have you given your application to your assessor to fill out the Assessor Report?
  7. Have you checked all the items on the Applicant Checklist in your application?
  8. Have you submitted your completed application?
  9. Congratulations!

Important information about benefits for Indigenous Peoples

If you are Indigenous there are many things that affect the benefits you can get and the dollar amounts you can get from them. These include the agreements your band or governing body has with provincial, territorial, and federal governments. Before applying to any benefits, you should speak with your governing body, if applicable.
