Here is how you can ask your medical professional to complete the Medical Report.

Section 2 starts with some directions and information for your doctor or nurse practitioner. They must fill out the part of Section 2 that begins on page 9. If you have more than one person who could help you, ask the person who knows you best to fill out the form.

Here are a few ways that you can help your doctor or nurse practitioner complete this section.

Make an appointment with your doctor or nurse practitioner to discuss the form. Section Two has many questions. How well your doctor or nurse practitioner can answer them will depend on how well they know you.

Take your application form with your section finished. Meet with them to talk about the form and go through the questions before they fill out their part. This is particularly important if you go to a walk-in clinic where they don’t know your health history well.

You may want to photocopy and fill out Section 2 yourself as a ‘draft’. You can consider getting some advice from an advocate before filling out a rough draft of section 2 for your doctor. Then you can show it to your doctor or nurse practitioner to see if they think it is accurate. If they agree with what you have written, they may want to use it as a guide. It will make their job easier and help them understand how your disability affects you.

FAQs

What if I do not have a doctor to help complete my application?

If you do not have a family doctor that you see regularly, finding one can be hard. You have the option to ask a nurse practitioner to fill out your application. Whether you get help from a doctor or a nurse practitioner, it is a good idea to see them a few times before you ask them to complete the form. It may help if they know you.

How to find a doctor or nurse practitioner who will work with you

Here are a few places to look:

Speak to your local pharmacy or hospital. Ask if they know of any doctors taking on new patients. If you live outside of Vancouver, you can use the Doctors of B.C. website to see if there are any new doctors taking on patients. Note: Sometimes the information on this website is out of date. Go to a walk-in clinic and try to see the same doctor each time you go there. Call the College of Physicians and Surgeons of B.C. to get information about finding a family doctor. Search for a doctor on the British Columbia Doctor Directory.

Do I have to pay to have my doctor, nurse, or assessor to fill out the application?

No, health professionals bill the provincial government for doing this. They should not charge you any extra fees.
