Step 2 - Persons with Disabilities designation (BC)

Do you have a Persons with Disabilities (PWD) application form?

You answered "no".

You answered "not sure".

There are four ways to get the application form:

  • You can have a paper application mailed to you.
  • Call the Ministry of Social Development and Poverty Reduction at 1-866-866-0800. You may have to wait a week or two to get your PWD application if you are getting it by mail.
  • You can find a ministry office in your community and go there to pick up a paper application.
  • You can request an application through your My Self Serve account, using a computer or mobile device. You can then download and print the application if you will be giving the other sections of the application form to the medical professionals to fill out.

How do I set up a My Self Serve account and request an application form?

To register for My Self Serve, you (and your spouse if you have one) will need to give your email address and Social Insurance Numbers. Click ‘Create an Account’ and follow the steps.

When you complete My Self Serve registration you will have:

  • a PIN number that will be used as your electronic signature and
  • a Basic BCeID User ID.

Once you are in My Self Serve, submit a ‘request for assistance’. Make sure to say that you want to apply for PWD status.

If you are Indigenous there are many things that affect the benefits you can get and the dollar amounts you can get from them. These include the agreements your band or governing body has with provincial, territorial, and federal governments. Before applying to any benefits, you should speak with your governing body, if applicable.
