The title of this webpage is Application for last-resort financial assistance or for employment services.

Under the question ‘Which type of application do you want to submit?’, select Application for last-resort financial assistance (social assistance).

Application for last-resort financial assistance (social assistance)

Documents and identification you will need to include with your application

You will be asked to provide certain documents for yourself and, if applicable, for each member of your family. The Ministère can ask you to provide additional documents to determine your eligibility and the amount you might get. Your application could be denied if you do not provide the documents in the time they ask for them.

You may be required to provide a document with your photograph issued by a public agency to verify your identity (for example, a birth certificate or something similar). You will need to go to one of the offices with your personal identification. If you are applying online using My Account-Employment Assistance-Social Assistance and are using clicSÉQUR, this step is not required.

If you were born in Québec

You do not need to provide a birth certificate unless the Ministère requests it.

If you were born in Canada, but outside Québec

You must provide the birth certificate issued by the office responsible for vital statistics in the province or territory of birth.

If you were born outside Canada

You must provide one of the following documents:

  • immigrant Visa and Record of Landing (IMM 1000);
  • confirmation of Permanent Residence (IMM 5292 ou IMM 5688);
  • document of decision granting refugee protection status;
  • document authorizing entry into Canada (IMM 1442).
Other documents you will need to provide

If you have health issues, a medical certificate attesting to your health condition or need for special equipment. You may also have to provide your Health Insurance Number.

Information on completing Section 1 of the Application for Last-Resort Financial Assistance

Section 1 is for your personal information and the personal information of another adult who meets the definition of your spouse.

Your spouse is (even if temporarily absent):

  • the person who lives with you and with whom you are married or in a civil union;
  • the person who lives with you and is the other parent of at least one of your children;
  • the adult person who lives with you in a conjugal relationship and who, at a given time, lived with you for at least one year.

Information on completing Section 3 – Income

Section 3 is to provide your income and the income of another adult who meets the definition of your spouse . Documents showing your current income or income that ended during the month of application (e.g., paycheque stubs, proof of payment of benefits or allowances, court decisions). Income can be from work or other sources.

Information on completing Section 4 – Liquid assets

Documents showing the value of your liquid asset (e.g, bank account statement, investment account statement (RESP, TFSA, RRSP, term deposits, cryptocurrencies).

Information on completing Section 5 - Property

Documents showing the value of your property and assets, including those held outside of Québec (e.g, vehicles, house (mortgage or loan statement), land, boats, life insurance).

Direct deposit

If you want your benefits from the Ministère de l’Emploi et de la Solidarité sociale to be paid directly into your bank account, you will select the Yes button and provide the following information:

  • Your blank cheque marked with the word “VOID” on it. If you do not have a blank cheque, you can ask your financial institution with a document the same as a blank cheque.
  • See the example below to find out where on your cheque to find some of the following information:
    • Name of your financial institution, address, and postal code
    • Your branch transit number
    • Your financial institution number
    • Your account number and designation number Your account number and designation number

Finalization of your application – Solemn affirmation

At the end of the application form you will sign your name affirming that the information you have entered into the application form is accurate and complete. The Ministère can ask you for any documents or information they need to confirm your eligibility and the amount of financial assistance you may get. They may also verify all your information by getting things such as your tax information for the previous year (and that of your spouse, if applicable) from Revenu Québec; information about your vehicles from the Société de l’assurance automobile du Québec (SAAQ); and properties from the Ministère des Affaires municipales et de l’Habitation.

At any time and without your consent, the Ministère can verify any information it has about you and your family with public or private organizations, landlords, employers, financial institutions, municipalities, credit bureaus, etc.

The Ministère can also use some personal information from the Québec Parental Insurance Plan (QPIP) or programs under its responsibility. This verification can include the cross-referencing of files.

Information on completing Section 1 – Reason for application

Check the ‘yes’ box for the question ‘Are you applying for last-resort financial assistance (social assistance)? and select the box which best describes the reason you are applying.

If you have dependent children

You must complete the Appendix-Dependent Children form (3003-03A) (PDF 501Kb).

Who is considered as your dependent

If you support them:

  • a minor child;
  • an adult child enrolled in an educational institution, either part-time or full-time.
Who is not considered as your dependent children
  • a minor child who is married or is the parent of a dependent child;
  • a fully emancipated minor child;
  • an adult child who is enrolled in an educational institution and who is married or in a civil union or is the parent of a dependent child;
  • a child who is sheltered in a rehabilitation centre or who is taken in charge by a foster family or an intermediate resource, unless the child’s return to your family is provided for in an intervention plan;
  • a child who is taken in charge by a tutor under the Youth Protection Act;
  • a child you had with a former spouse if this spouse supports the child.

Tip

Make sure you save the current online form (if completing the application form online) to avoid losing your data. To save your information, click the “Save” button at the bottom of each online form page. If you complete your application in the hardcopy form, make a photocopy of the application forms before sending them to the office.


FAQs

What is the Application for employment services, and can I select it also to complete?

If you want to start working, the application for employment services allows you to take advantage of measures or services to help you with entering the labour market, depending on your situation. You can apply for employment services and for last-resort financial assistance at the same time, by clicking the box for both applications.

Who can I call to get help filling out the application form for the Social Solidarity Program and to answer my questions?

To get help filling out the form or to get a document regarding health issues, you can call the Centre de communication avec la clientèle at 1-877-767-8773 (toll free) or go to one of the Services Québec offices.

Who is considered a member of a visible minority?

A member of a visible minority is a person, other than Indigenous, who is easily recognizable as having a different ethnic identity or skin color.

Can I get reimbursement for funeral expenses for my family?

If you paid funeral expenses for your spouse or family member who is deceased, you can apply for reimbursement of those expenses. Use the same form, Application for Last-Resort Financial Assistance (3003-02), to submit for reimbursement. Along with your application form, you must also provide the following:

  • Attestation or Declaration of death or a copy of it
  • Documents showing the person’s account balance on the date of death
  • If you have already received it, the notice of decision from Retraite Québec about the death benefit
