Step 2 - Social Solidarity Program and Basic Income Program
Do you have the application form to apply for financial assistance?
You answered "no".
Once you have confirmed that you may be eligible for financial assistance, you will need to fill out some forms These forms are called Last-Resort financial assistance (social assistance).
There are 2 ways to access your forms:
- You can access and complete the forms online at Apply for Social Assistance Online.
- If you don’t want to submit your application online, you can also submit your application by filling out the following forms:
- Application for Services (3003-01A) (PDF 667 Kb)
- Application for Last-Resort Financial Assistance (3003-02A) (PDF 1.11 Mb)
- If you have dependent children, you must complete the Appendix-Dependent Children form (3003-03A) (PDF 501Kb)
FAQs
Who can I call if I have questions about the application forms or get the forms by mail?
You can call toll-free to the Centre de communication avec la clientèle at 1-877-767-8773 to find out what forms and documents you will need to apply for the Social Solidarity Program or how to fill out the forms. You can also ask to get the forms by mail if you cannot get them at the Services Québec office.
Do I need an application form to apply for the Basic Income Program?
There are no steps you need to take to get the Basic Income Program benefits if you are eligible. If you meet the following requirements, you will be admitted into this program automatically:
- You are already receiving benefits under the Social Solidarity Program.
- You have had severely limited capacity for employment for at least 66 months over the previous 72 months.
