Here is the information you will need to complete the Medical Report form

The Medical Report form will need to be completed by your physician to indicate that you have a severe medical condition that prevents you from being able to work full-time. Your condition must also be permanent, which means it is of an indefinite duration with no possibility of improvement.

You will need to complete section 1 of your application. Your doctor will fill out the rest of the application. They will then send it to Retraite Quebec at the following address:
