Step 3 - Disability Benefits under the Quebec Pension Plan

Here is the information you will need to send in the Application for Disability Benefits under the Quebec Pension Plan

Your application should be submitted as soon as possible. The maximum retroactivity possible is 12 months from the day your application is received. You will need to complete additional steps, but these can be completed after your form has been submitted. In some cases, you may also be able to receive QPP while you are waiting for your disability benefits to be processed.

You can submit your application two different ways: by submitting your form online or by mailing your application to Retraite Quebec.

The first option to submit your application is to submit your form online. To submit the form online, save the document to your computer and then upload it as instructed.
