Once you have downloaded your Application for Disability Benefits Under the Quebec Pension Plan, you will need to complete the application by typing into the form. Read all of the information carefully and make sure to include all relevant information.

This form should be submitted as soon as possible. The maximum retroactivity possible is 12 months from the day your application is received. You will need to complete additional steps, but these can be completed after your form has been submitted. In some cases, you may also be able to receive QPP while you are waiting for your disability benefits to be processed.

Additional payments for children

If you have children, they could also be entitled to a pension for a disabled contributor’s child until they reach the age of 18. Your child could be eligible if they:

  • Are your biological or adopted child
  • Have been living with you for at least one year at the time you are deemed disabled and you are their mother or father
