Step 5 - Ontario Disability Support Program

Here is the information you will need to meet with the ODSP office to sign and verify your application

When your local ODSP office contacts you about your ODSP application, you will schedule an appointment to meet with an intake caseworker for your verification appointment. During the verification appointment, the intake caseworker may ask you to provide more information to verify the information you provided in your online application, such as copies of bills or other types of documentation.

You will also need to sign your application package and complete a consent form. The consent form will allow ODSP to access information about you from other government programs to determine if you qualify to receive ODSP, such as the Ministry of Transportation, or Employment Insurance.

If you are Indigenous there are many things that affect the benefits you can get and the dollar amounts you can get from them. These include the agreements your band or governing body has with provincial, territorial, and federal governments. Before applying to any benefits, you should speak with your governing body, if applicable.
