Here is the information you will need to select that you consent to the release of your information from your income tax returns and other required taxpayer information

For your application to be reviewed, your income tax returns and other required taxpayer information will need to be provided. You have the option to decide whether you consent to having this information released from the Canada Revenue Agency (CRA) to an official of the Government of Northwest Territories Income Security Programs or whether you will provide the necessary information.

You must select one of these options and provide your signature. If you have a co-applicant, they will need to provide their signature too.
