Here is the information you will need to submit your completed application, including your financial assessment and Certificate of Disability form

Once you have completed your application with help from your Income Assistance worker, you will submit your completed application, including all necessary documentation such as:

  • The Certificate of Disability form
  • Proof of employment records
  • Any other documentation your Income Assistance worker requests or additional financial supports through Income Assistance.

If you are Indigenous there are many things that affect the benefits you can get and the dollar amounts you can get from them. These include the agreements your band or governing body has with provincial, territorial, and federal governments. Before applying to any benefits, you should speak with your governing body, if applicable.
