You will need to submit a Certificate of Disability form, which your healthcare professional will complete. This could be a physician, nurse practitioner, or psychologist.

Benefits of Qualifying as Disabled

Typically, people who receive Income Assistance are required to look for employment and participate in programs that could help them find employment. As a person with a disability, you may be exempt from this requirement.
