Here is the information you will need to collect the required documentation for your first meeting with a caseworker

To complete your application with a caseworker, you will need to provide copies of specific types of documentation. The caseworker will make photocopies of your documentation to keep in your file. The required documentation could include:

  • Your Social Insurance Number (SIN)
  • Driver’s license or photo ID
  • Bank account statements
  • Income tax assessment forms
  • A copy of your lease
  • Bills or receipts for utilities
  • A medical report from a physician or nurse practitioner who is familiar with your medical history

You should collect this documentation prior to your first meeting and bring it with you when you meet with your caseworker.

If you are Indigenous there are many things that affect the benefits you can get and the dollar amounts you can get from them. These include the agreements your band or governing body has with provincial, territorial, and federal governments. Before applying to any benefits, you should speak with your governing body, if applicable.
