Here is the information you will need to ensure the AISH Application – Medical Report form is submitted
As part of your AISH Application, your physician will need to complete the AISH Application – Medical Report. This report will include:
- Your basic information
- Your physician’s relationship to you
- Your diagnosis(es) and relevant details about the diagnosis(es)
- Your medical history
- Your degree of impairment
- Your medications, if applicable
- Your treatment, if applicable
- Your prognosis
- Any additional comments or information
- Your physician’s certification
Submitting the Medical Report
If your doctor has provided the AISH Application – Medical Report to you, you can submit it with your online application. Your doctor can also submit your medical report by fax or mail to the following address:

If there is a charge to have your physician complete the Medical Report, you are responsible for paying it. If you are currently receiving Income Support, you may be eligible to have the cost covered. You can contact your Income Support worker to see if you are eligible to have this cost covered.
Unique Situations
There are some unique situations that will not require you to complete the AISH Application – Medical Report. If you are in one of the following situations, you may be required to provide more specific medical documentation instead.
End-of-Life or Palliative Care
If you are receiving end-of-life palliative care or have been diagnosed with a terminal illness, you will not need to provide the AISH Application – Medical Report. Instead, you will need to provide medical documentation that shows you have a terminal illness or are receiving palliative care.
Long-Term Care or Designated Supportive Living
If you were assessed as needing long-term care or designated supportive living, you will not need to provide the AISH Application – Medical Report. Instead, you will need to provide medical documentation that shows you need long-term care or designated supportive living.
Applied for Persons with Developmental Disabilities (PDD) Program
If you applied for the Persons with Developmental Disabilities (PDD) program, you will not need to provide the AISH Application – Medical Report unless you are contacted by an AISH worker.
