Step 2 - Assured Income for the Severely Handicapped

You answered "no".
Here is the information you will need to complete the AISH Application – General Information form or the online application
Remember, you have the option to fill out the PDF application for AISH or the online application to complete this portion of the AISH application.
This section of the application will ask you questions about some of the following information:
- Your personal information (example: you name, date of birth, address)
- Your spouse’s information (if applicable)
- Information about any dependent children you have
- If you have a trustee or power of attorney
- Employment history
- Education or training history
- Income
- Assets
Make sure to provide the most accurate information possible. If you need to collect more information or take a break, you can save your online application and exit at any point and return at a later date.
Completing Your Application in Unique Situations
There are some cases where you will not be required to complete the entire application or you may need to provide additional documentation. See below for a list of unique situations that may impact how you complete your application. If none of these situations apply to you, you should complete all sections of the PDF application for AISH or the online application.
End-of-Life Palliative Care or Terminal Illness
If you are receiving end-of-life palliative care or have been diagnosed with a terminal illness, you can skip Section 5: Employment History and Section 6: Education and Training History. You will also need to provide medical documentation that shows you have a terminal illness or are receiving palliative care. This medical documentation will replace the Medical Report described in Step 3.
Long-Term Care or Designated Supportive Living
If you require long-term care or designated supportive living, you can skip Section 5: Employment History and Section 6: Education and Training History. You will also need to provide medical documentation that shows you need to live in a long-term care facility or a designated supportive living facility. This medical documentation will replace the Medical Report described in Step 3.
Currently Receiving Income Support Benefits
If you are currently receiving income support benefits from Alberta Works, you will not need to provide your financial information again. This means you can skip Section 7: Income Information and Section 8: Asset Information.
Applied for Persons with Developmental Disabilities (PDD) Program
If you have applied for the Persons with Developmental Disabilities (PDD) program, you will not need to have the Medical Report explained in Step 3 completed unless you are contacted by a AISH worker.
Left the AISH Program Less than Two Years Ago
If you left the AISH program less than two years ago, you should contact an AISH office to see which sections of the application you will need to complete.
