Step 3 - Income Assistance

Here is the information you will need to attach all necessary attachments to your Application for Income Assistance

For a number of the steps in the Application for Income Assistance, you are asked to attach documents if they are relevant to you and your current situation. This information will allow your application to be properly assessed. If these documents apply to you, make sure to attach them to your completed application!

The necessary documents (if applicable to you) include:

• Copies of your Record of Employment(s) for the last six months
• An extra sheet if necessary to list all member of your household
• Your lease agreement
• Your official mortgage and the most recent statement and receipt of payment
• Copy of your rent-to-own agreement and the most recent receipt of payment
• Verification of all income received
