This is necessary because the Department of Social Development will ask you for this information when you contact them.

This includes the following information about yourself:

  • Your mailing address
  • Your residence address if it is different from your mailing address
  • Your rental or mortgage cost

This will also include the following information about everyone living in your household, including yourself:

  • Their first and last name
  • Their birthdate
  • Their Medicare number
  • This Social Insurance Number (SIN)
  • Their income
  • Their assets
