Here is how you can find out how to get a copy of the CPP-D application form and more information about completing it.

You can get the CPP-Disability application form three ways:

  1. Request a paper copy from Service Canada at 1-800-277-9914.
  2. Download and print the CPP-D non-terminal illness application form or the CPP-D Terminal illness application form.
  3. Apply online by registering for a My Service Canada account (MSCA). Log into your account and complete the application form. Note: You will still need to sign and mail a Consent for Service Canada to Obtain Personal Information form (ISP-2502).

When should I apply?

The CPP-Disability application process has two parts: the application form and the medical report. You should complete the application form as soon as possible after you become disabled. You do not need to wait for your doctor or nurse practitioner to complete the medical report before sending in your application. The date that Service Canada receives your application could affect when your benefit starts.

Which application form should I use?

If you and your doctor do not expect you to live longer than six months, use the CPP-D Terminal illness application form.

Service Canada has a list of grave medical conditions. These are conditions that progress rapidly. If you have one of these, but you are not expected to die within 6 months, use the CPP-D non-terminal illness application form. State on the form that you have a grave medical condition. Service Canada will process your application faster.
