Step 4 - Persons with Disabilities designation (BC)

Here is how you need to complete Section C.

In Section C, sign your name and put the date on your form.

If a person is not able to sign the application because of mental disability, it can be signed by someone who has the legal authority to sign on their behalf.

You are declaring that the information you provided in the section 1A and 1B are true and complete. You are also saying you understand that you will be able to review the completed section 2 (Medical Report) and section 3 (Assessor Report) before you submit your completed application form. You are also agreeing that you understand that the BC government may verify the information provided in all the application sections in order to determine and confirmation your eligibility for the Persons with Disabilities designation.
